2013 Indoor Program Information
WSTP is offering FREE tennis instruction to west suburban youth (age 10 to 17) at the Courts Plus, Elmhurst Park District (186 South West Avenue, Elmhurst, IL 60126) on various Sunday evenings starting January 27, 2013 through April 28, 2013.The age range of participants is limited due to reduced court space availability. There are different start and end times per lesson for different age groups. All participants must have a signed "Parent/Guardian Permission Form".
REGISTRATION INSTRUCTIONS:
- Parents/guardians should download this schedule and fill out this permission form. Parents/guardians must sign the permission form digitally or offline.
- The free Adobe Reader software is required. If your computer doesn't have this software installed,
. - Form field content can't be saved, so it will be necessary to print the permission form after filling in the fields.
Then, please date and sign the form. - Fill out a permission form for each child. Each child must bring the form to the first attended class.
